(2) Submit Your Medical History

This quick-start, online Step is optional. But, this process is convenient, time-saving, effective...and necessary. Please click on the purple Med Info button to use our logic-driven, self-paced system for your medical history and current symptoms. When you are finished, you can print out a summary of your information. Because it is securely stored online, your history/symptoms will be reviewed before you arrive at Brooklyn Heights Family Practice’s. We will not save this information in your Electronic Health Records until reviewed and modified during your visit. Unused and/or preliminary information will be discarded to protect your privacy.

(3) Message With Your Team

Our secure messaging system—part of HIPPA-compliant Practice Partner—is a Web-based service that enables my staff or me to exchange with you in safe, private online correspondence. [Please note online registration for Practice Partner is not yet available. For the interim, you may register by telephoning us or by coming into the office.]  With your registration-provided User Name and Pass Word, please log-in at Practice Partner using the green Messaging button. "Messaging" is the first function-set in the Left-Sidebar Navigation Menu. Use the "New Message" link to send us secure messages. The system will notify you using your regular email address when our response(s) are available to you (via your "Inbox" back on secure Practice Partner).

(4) View Your Chart

Our secure chart system—part of Practice Partner—is a Web-based service that enables you to peruse the contents of your own chart. For the interim, you may register by telephoning us or by coming into the office.]  With your registration-provided User Name and Pass Word, please log-in to secure Practice Partner using the red View Chart button. "Chart" is the second function-set in the Left-Sidebar Navigation Menu.
